Key Summary

Botox is considered when enlarged chewing muscles make the lower face look wide. Jaw contouring surgery is considered when the desired change involves the jawbone's angle or width. Both need examination when muscle and bone contribute. A muscle bulge when clenching is a useful clue; appearance alone does not determine whether surgery is appropriate.

The jawline changes over several weeks after Botox, and maintaining the effect involves repeat treatment. Reviews describe a slimmer appearance, small or short-lived changes, and weaker chewing. Surgery offers a lasting change to bone shape but involves anesthesia and greater recovery demands. Swelling, difficulty eating or opening the mouth, and changes in sensation need attention after surgery. Returning to work and reaching a settled appearance after residual swelling subsides are different milestones.

How can you tell whether muscle or bone causes a square jaw?

The clinician examines the muscles felt during clenching and the shape of the jawbone, with imaging as needed when surgery is considered. The firm chewing muscle near the ear is the masseter. A thick masseter can make the lower face look wide.

Hyundai Mihak Plastic Surgery Clinic explains that muscle development and left-right differences guide injection placement and dose. Asan Medical Center describes preoperative X-rays to assess bone and muscle, together with an assessment of how the teeth meet.

Area to examineWhy it matters
Muscle that bulges during clenchingAssessing how much muscle is available to reduce with Botox
Jaw angle near the ear and lower-jaw widthEstablishing whether the desired change involves bone shape
Cheek volume and sagging skinAssessing whether reduction could leave a more hollow or sagging appearance
Bite and jaw-joint symptomsIdentifying other dental or jaw-joint problems

Feeling a muscle when clenching does not mean Botox is necessary. Explain the change you want and discuss an approach based on the examination.

What do Botox and jaw contouring surgery each change?

Botox is injected into muscle; square-jaw contouring surgery reshapes bone. Botox temporarily reduces nerve signaling to the muscle, allowing its volume to decrease gradually. Surgery approaches the jaw through the mouth and removes part of the jaw angle or outer bone layer.

Facial contouring can include cheekbone and chin surgery as well as jaw-angle reduction. When a review describes a change to the whole face, check which procedures were combined.

What benefits and disappointments do Botox reviewers describe?

Accounts include a slimmer jawline as well as small changes after one treatment. One Gangnamunni reviewer described noticing a change around two weeks after treatment.

효과는 2주 뒤 부터 보였던 것 같아용

English translation: I think I started seeing the effect after two weeks.

Another reviewer felt the first treatment made only a small difference and also expressed hope about further sessions.

효과가 미미하게나마도 있는 듯합니다.

English translation: There seems to be at least a slight effect.

Accounts of immediate swelling or injection pain do not establish how the jawline looks months later. Check when post-treatment photographs were taken and compare their angles. Clinic names are omitted from the platform excerpts, which are presented separately from clinic guidance.

When do results appear, and how long do they last?

Changes develop gradually. Hyundai Mihak Plastic Surgery Clinic describes jawline changes over four to six weeks and effects lasting about three to six months. Some reviewers report a shorter duration than they expected.

2개월 정도 지나니까 효과가 미미해졌어요!

The next treatment is decided by remaining muscle size, chewing strength and the response to previous injections, rather than the calendar alone. The clinic advises an interval of at least three months. If the effect seems weak, provide the product, dose and treatment date before discussing whether another injection is appropriate.

Units describe the dose of a botulinum toxin product. Ask which product is used and whether the quoted dose is for both sides together or for one side. The Botox dose guide explains product and dose considerations.

Can Botox weaken chewing or make the cheeks look hollow?

Because treatment reduces a chewing muscle, chewing strength, hollowing, asymmetry and expression changes all need consideration. The injection location and dose deserve particular attention when cheek volume is already low.

A reviewer who received injections in both the jaw and temples described weaker chewing.

Gangnamunni, combined jaw and temple Botox review, displayed August 12, 2026, excerpt, search original reviews

씹는 힘이 진짜 약해짐

English translation: My chewing strength really weakened.

A study following 30 people found that maximum bite force was about 20% lower two weeks after the first injection. It then recovered, returning to the pretreatment level by week 12. The discomfort people experienced varied. Bite-force study

Researchers have also examined jawbone changes after repeat injections. In a 20-person study, bone volume around the jaw angle decreased more in the group receiving two treatments. In contrast, a one-year study of 187 people found no difference in measures including bone width and thickness compared with injections containing no active drug. The effects of repeated treatment over many years remain uncertain. 20-person study, one-year study

If chewing becomes difficult or your smile looks asymmetric, contact the treating clinic before further injections. Mention grinding, clenching and previous injections into other chewing muscles.

How long does swelling last after surgery, and when can you work?

Resuming basic daily activities and having substantially settled swelling are different milestones. One jaw-contouring reviewer described marked swelling for the first two weeks, followed by a more natural appearance.

2주동안은 붓기가 심해서 마스크 착용하고 있었고

English translation: For two weeks, swelling was severe, so I wore a mask.

The reviewer wrote that others commented on a more natural appearance at about six weeks. The linked product is a three-area facial-contouring package, so check combined procedures before comparing this account with jaw-only surgery.

If your work involves meeting people, discuss leave that accounts for visible swelling and mask use. Food and oral care also change while the incision heals. Asan Medical Center advises liquids initially, followed by soft foods such as porridge and a gradual return to a normal diet according to recovery.

How long can tingling or reduced sensation last?

Recovery of sensation depends on the surgical area and the condition of the nerves. Braun Plastic Surgery Clinic describes improvement in temporary sensory loss over one month to one year. A Babitalk reviewer who combined cheekbone, chin and jaw surgery wrote the following at seven months.

Babitalk, review 4916176, seven months after combined cheekbone, chin and jaw surgery, excerpt, search original reviews

턱끝에 아주 살짝 저림이 있긴 한데 크게 불편하거나 그렇진 않아요

English translation: There is a very slight tingling at the chin, but it is not particularly uncomfortable.

Asan Medical Center lists altered sensation from nerve injury, bleeding, hematoma and infection among surgical complications. A hematoma is a collection of blood within tissue. Have persistent tingling assessed rather than using another person's review to determine its cause or recovery time.

Ask where the nerves run inside the jawbone, how much bone will be reduced and how sensory changes will be assessed after surgery. When several areas are treated, ask for an explanation of symptoms expected in each area.

If Botox makes little difference, should you have surgery?

First have the unchanged area reassessed. A prominent bone angle remaining after muscle reduction differs from a small response in the muscle itself. A limited result does not establish that bone is the cause or that resistance has developed.

Some operations also reduce muscle. One Percent Plastic Surgery Clinic describes combining jawbone reduction with masseter reduction and removal of fat inside the cheek. Ask which elements your own face needs. Reducing bone, muscle and fat together is not necessary for everyone.

When cheek volume is low or skin already sags, hollowing and the jawline need assessment alongside width. Braun Plastic Surgery Clinic discusses postoperative sagging in people with reduced skin elasticity. For a separate discussion of sagging, see the facelift and mini-facelift guide.

Are contouring injections the same as Botox or surgery?

Contouring injections, Botox and facial contouring surgery are different treatments. The name “contouring injection” alone does not identify the ingredients or treatment scope. An injection intended to treat fat around the jaw has a different purpose from muscle-targeted Botox or bone surgery.

Ask which tissue a treatment addresses, rather than relying on a promise of a smaller face. Check the drug, ingredients, amount and number of sessions, and distinguish these injections from treatments for muscle or bone.

What should you compare about anesthesia and cost?

For Botox, compare the product and dose. For surgery, compare the exact scope and whether anesthesia, admission and aftercare are included. One Percent Plastic Surgery Clinic describes general anesthesia and either same-day discharge or one night in hospital for its jaw operation. Asan Medical Center also describes mandibular angle reduction under general anesthesia. Confirm the arrangements for your planned operation.

Consultation topicQuestion to ask
Botox product and doseWhich product will be used, and what is the total dose for both sides?
Repeat treatmentWhen will you assess my response and decide whether to repeat treatment?
Surgical scopeWill you reshape the jaw angle, outer bone layer or both? Are chin or cheekbone procedures included?
Muscle and fatWhy are masseter reduction or inner-cheek fat removal being added?
Anesthesia and admissionWho handles anesthesia, how long will I stay, and when will I leave?
Additional chargesDoes the total include tests, anesthesia, admission, medication, aftercare and VAT?
Follow-upWho assesses sensory changes or asymmetry, and how are related costs handled?

A reviewer's payment and a promotional package price are different figures. Jaw-only surgery also differs from a package including cheekbones and chin. Compare totals for matching procedures and included items.

What should you discuss first for your situation?

Four consultation topics covering clenching-related muscle bulging, jawbone shape, combined muscle and bone development, and low cheek volume or sagging
Figure 2. Clues to discuss during examination. The topics are not a required sequence of procedures or tests.
Current concernWhat to discuss
A large muscle bulge when clenchingMuscle thickness, left-right differences and expected Botox changes
A prominent jaw angle or width at restThe bone area you want to change and any necessary imaging
Both muscle and bone are enlargedTheir respective contributions, your goal and treatment order
Low cheek volume or saggingHollowing and sagging after reduction, and areas to preserve
Small changes despite repeat BotoxPrevious products, doses, intervals and remaining muscle size
Limited time off workSeparate timelines for work, swelling, eating and exercise

Conclusion

Begin by establishing whether the area you want to reduce is muscle or bone. During consultation, describe your desired front and side views and ask about expected changes, chewing function and recovery. If you have had Botox, provide the product, dose and treatment dates to help avoid unnecessary additional injections or surgery.

Frequently Asked Questions

Can clenching tell me whether I have a muscular square jaw?

It shows how much the masseter bulges, but does not by itself determine treatment. Have muscle, bone, cheek volume and skin assessed together.

Does masseter Botox make the jawbone smaller?

Masseter Botox is intended to reduce chewing-muscle volume. It is not presented as a treatment that directly reshapes the angle or width of the jawbone.

Do I have to repeat jaw Botox every six months?

Repeat treatment is not mandatory on a fixed date. Remaining effect, muscle size and chewing strength guide whether and when to repeat it.

Should I increase the dose if the result seems weak?

Have muscle size and your previous response assessed before another injection. The clinician also considers bone and fat contributions when adjusting the product and dose.

If I can work after surgery, has all the swelling gone?

Work readiness and resolution of swelling are different criteria. Discuss residual swelling when your job involves meeting people or you have an important photography session planned.

Is jaw surgery the same as three-area facial contouring?

Jaw surgery addresses the lower jaw. Three-area contouring usually refers to cheekbone, jaw-angle and chin surgery together. Check the actual components in the quote and surgical plan.

Should I mention previous Botox at a surgical consultation?

Provide the product, dose and last treatment date. The clinician considers current muscle size and remaining effects when discussing the need for surgery and its timing.

Can I have square-jaw treatment if my cheeks are already thin?

The clinician first examines areas to reduce and volume to preserve. Cheek hollowing and sagging are considered when choosing the Botox dose or surgical scope.
